The Frog has two life... does damage from the player transfer to the frog and then back to the player? Does the Frog count as a player on the team (thus potentially preventing Hail Marry)? Can the player of the team activate the Frog for their turn? Does the frog take up a spot on the board?

Also just to confirm, it triggers when you LOSE a challenge? I'm pretty sure it said WIN a challenge during the kickstarter previews.

Yes, the damage transfers, it is similar to the Lycanthropes changing form.

Does the Frog count as a player on the team (thus potentially preventing Hail Marry)?

My understanding is yes, because the frog can still be attacked or tackled. It just can not initiate the contest.

Can the player of the team activate the Frog for their turn?

Again, my understanding is yes, if you want your frog to run far far away from the action it can.

Does the frog take up a spot on the board?

Due to my answers above I would most definitely say yes.

Correct it is when you lose a contest. Also correct in saying that the power was different during the campaign.
