That is the incredibly nasty 'reverse barb' spear-head. A barbed head hooks in, and tears the skin outward when pulled out. The reverse barb (read: scoop) makes a clean hole, but leaves no flaps for surgeons to sew back together. The deeper the spear goes, the bigger the hole made, and the harder to heal, if the victim even survives.
