The head is from a 5th edition Flamer of Tzeentch.
Quite a simple conversion really, take the head from the flamer... cut the horns from the minotaur head and they fit directly into the arm slots of the flamer. a pin and liberal use of some green stuff around the neck.
All in all an effect that I was very happy with - and fitted perfectly in with the theme of the army.
